All prospective athletes must complete the following steps in order to participate in athletics, marching band, JROTC, dance, or color guard.
Step 1: Create Your Athletic Clearance Account

Click the link above to create your account, select the school year and Innovation (Orlando) High School. Select every sport/activity you would like to participate in. There is no limit to how many sports you can select. Read the instructions on the website carefully and please complete all sections. This website is the where you will upload you sports physical (EL2) and EKG/ECG, and other documents needed to participate in high school athletics/activities.
Step 2: Sports Physical (EL2)

Download the sports physical packet and take it to your sports physical doctor appointment. Please read this form carefully and fully complete each section. Please make sure that each page is fully completed and all parent/guardian, student, and doctor signatures are legible. The packet must include the doctor's stamp that performed the physical. Only page 4 is required to be uploaded. Physicals are required yearly and are good for 365 days from the date of the exam.
Step 3: ECG/EKG

An ECG/EKG is required only once for high school. Please read the form carefully and make sure all signatures and stamps are present.
Step 4: NFHS Courses

Three courses are required yearly to participate in sports. The courses required are Concussion for Students, Heat Illness Prevention, and Sudden Cardiac Arrest. These classes are free and are required to be taken by the student. When signing up make sure you use your full legal name, select Florida, and that you are a student. Each class takes about 30 minutes to complete and at the end of each course you will be given a completion certificate. This certificate is uploaded into the athletic clearance website. The courses are also accessible via links on the forms section of the athletic clearance website.
For athletes who wish to participate in Summer workouts, or off-season workouts, please download, complete and submit the Summer/Off-season Release Waiver.
