Physical Packet
All athletic physicals are now being submitted digitally via www.athleticclearance.com. This digital platform will allow parents and student athletes to complete and access forms via computer, tablet, or mobile phone. It is HIPAA, COPPA, and FERPA compliant. This process will be linked to FHSAA Home Campus which will be automatically updated once registration is complete. All athletes for sports in the 2024-2025 school year will need an EKG in order to participate. You only need ONE EKG for all 4 years at Innovation High School. Incoming freshmen, do NOT get your EKG done until AFTER April 1st. If you have it done prior, we cannot accept it!
